Privileging Forms

Would you be willing to share your Privileging forms for your LIPs? I am trying to determine what is the best method and, I am not sure ours is the best process. We have them done at the time of hire and upon recredentialing, but the form, doesn't seem "politically correct". We send the form to the candidate who completes column 1, and a verifying physician is to complete column 2 and then our Medical Director does column 3 saying they recommend them for the following privileges, but we have not yet been shown they can/can't do these duties. What if they are new and have not practiced, I know our requirements are to verify using training, education, etc. So, what do you do for initial hire vs recredentialing? I am attaching a sample for one of our providers that we currently use, but I had a brand-new NP, who had nobody to sign as a verifying physician, so this triggered the question of how we do this for the new employees who we have not even had work one day with us to confirm/deny yet.
