Description: The MAP framework - Measure Accurately, Act Rapidly, and Partner - with patients has proven effectiveness in producing a rapid and sustained improvement in blood pressure control in primary care settings. This presentation focuses on the Act Rapidly portion to promptly ensure an adequate treatment plan when blood pressure is verified to be uncontrolled. Success at this step can greatly facilitate a large and sustained improvement in blood pressure control for the patients served.
Learning Objectives:
- Define therapeutic inertia and list contributing factors
- Describe impact and importance of treatment intensification on blood pressure control
- Review strategies to address therapeutic inertia and increase treatment intensity, including single-pill combinations of antihypertensive medications
- Understand the need to achieve mean systolic blood pressure values ≥ 10 mmHg below goal to achieve high and consistent levels of blood pressure control
