Medically Tailored Meals Program Webinar

MACHC, in partnership with the Maryland Department of Health, Food & Friends, and Moveable Feast, will host an informational webinar on the Medically Tailored Meals program, an MDH initiative that provides patients with diabetes access to medically tailored meals and a nutritionist for six months. The webinar will inform FQHCs about the program and explain how to refer your patients. This is a great opportunity for providers, nurses, pharmacists, CHWs, or anyone else on your quality and population health teams who could refer patients to receive medically tailored meals. Please share with all relevant staff.
