CMS Requires All Permanent and Seasonal Sites to Use a Unique Medicare Billing Number

Urgent Information!


Attention all Health Center CEOs and CFOs:

It has come to our attention through HRSA that there may be some discrepancies in the Medicare numbers that have been submitted on Form 5B in EHB. This could have reimbursement repercussions if these billing numbers are not accurately listed in EHB.

According to PAL 2011-04:



  • ALL sites within a Health Center’s approved scope of project are required to enroll individually in Medicare.

  • Each site needs to be listed on Form 5B in EHB.

The Bureau recommends that all Health Centers review all Medicare billing numbers, ensuring that they are correct and that each site has a number assigned to it.

Please be sure to check these numbers at minimum, annually, to safeguard your Medicare reimbursement.
